Below are most of the requisite enrollment forms, and are separated into specific packets. Not all students will require all packets. In an effort to expedite the process, these forms are made available for completion now. You will still need to bring them to the HS Cafeteria (one location this year) on Thursday, Aug 3, from 7am to 6:30pm to complete the process. Many of the past years' individual forms have been combined into single sign-off sheets for your convenience, however, state regs still mandate a number of individual forms be completed yearly (sorry 'bout that).
Upon entry to the Cafeteria, you will be met by our staff members and given some additional forms that are not available at this time. Returning families will be asked to review their past information for accuracy (instead of having to complete another form), corrections being made at that time. Once completed, you will need to visit Mr Newton's and Mrs Cink's (school nurse) table. PRIOR to checking in with these stations, the Free/Reduced Meal Application and all Health Forms are to be completed for review at these tables. Jr & Sr High students will need to attend and visit with Mrs Fesmire and Pecha regarding class schedules.
Assistance will be available for any questions with form completion.
re: MEAL SERVICE......several items: 1. about the FREE / REDUCED APPLICATION. We understand the sensitivity of this issue & information on several levels. We also need to share that it is VERY important to the District that they be completed, as they net us significant funding in a number of areas. In an effort to ensure your confidence in maximum confidentiality and accuracy, all forms will be processed in the Administrative Office this year. In addition, the only information that is forwarded to any gov't agency is the number of apps received and the final determination count (not names or info) as to approved or not approved. Your Personal information is NEVER sent out, and if approved, there is absolutely no difference in the way it is applied to your student(s). There is no way anyone knows whether they are on Fr/Red or not.
ALSO, as stated at the end of last year, the District is looking at trying to provide at least free breakfasts for our students. This will require all families to complete the app and have a positive account balance. All factors in this decision (these two plus the Oct 1 State funding decisions) will be used to determine if this option is viable.
2. As per last year's Board of Education Meal Policy revision, ALL accounts now must carry a positive balance. The District will no longer be carrying negative balances.
OUT--OF--DISTRICT Information__Please be familiar with Tlake's Out-Of-District policy. Along with appropriate STATE form (below), an additional DISTRICT form is to be completed and submitted to the Principal or Administrative Office for consideration. All OOD transfers require Board of Ed approval before attendance at TLake unless otherwise indicated.
State Forms: Open Transfer(if submitted prior to May 31 for next year). Emergency Transfer for dates after May 31 An Emergency Trans request will only be good for that current year. Either an Open form needs to be approved for the next year prior to May 31 or a new Emergency form needs to be submitted and approved after May 31.
