**Please read information carefully. Below you will find most of the requisite enrollment forms that have been put together into family packets. In an effort to expedite the process, these forms are made available for completion now. Many of the past years' individual forms have been combined into single sign-off sheets for your convenience, however, state regulations still require a number of individual forms be completed yearly. Forms need to be turned in at one of the following enrollment sessions (ALL PK-12 students welcome at either session): Tuesday, August 4, 2020 Elem Cafeteria-Jet Noon to 6:30 pm Wednesday, August 5, 2020 HS Library-Helena 8:00 am to 2:00 pm Jr & Sr High students will need to attend and visit with Mrs. Pecha regarding class schedules.
Timberlake Public Schools is providing the option for a full, at-home virtual school program for families with Covid related health concerns. This path does not allow for participation in extracurricular activities and is a priviledge that comes with strict attendance and educational requirements. If you are interested in this path, please read the Timberlake Virtual Academy handbook below and return the application to kpierce@tlake.k12.ok.us as soon as possible or at enrollment if you choose this path.
Packets include medical and other forms that may not be applicable to every student including safety information for grade 5-12 athletes and drug testing paperwork for 9-12 students that plan to participate in extracurricular activities. There is also a form from the Oklahoma State Department of Health that is not mandatory. This form allows the health department and administration to communicate openly regarding Covid-19 and the safety of our students. While it is not required, please keep in mind that this flow of information can help keep school in session longer, provide us with the information needed to keep students safe, and allow potential quarantines to be less disruptive by making the contact tracing procedure more efficient.

NEW Families: Families that live outside the Timberlake School District MUST be approved for transfer prior to enrolling. Directions below. -in addition, you will need the student's Birth Certificate and Current Immunizations.
================================================================== Timberlake Out of District Policy The following forms must be completed and returned to the Timberlake District Office to be considered for an out of district transfer. Transfers must be approved by the Board of Education prior to being officially enrolled at Timberlake unless otherwise indicated. Timberlake reserves the right to limit out of district transfers based on actual or anticipated in-district enrollment numbers. Class sizes will be maintained at an optimal size in an effort to provide the most effective educational environment possible.
================================================================== MEAL SERVICE: Timberlake Public Schools is very interested in finding a way to at least provide free breakfast to all PK-12 students. For this to be possible, it is crucial that we substantially increase the number of Free and Reduced Applications turned in. We understand the sensitivity of this issue, but it is VERY important to the District that they be completed, as they net us significant funding in a number of areas. Many times, families are surprised that they qualify for at least reduced meal prices. Even though you may not need help paying for meals, it will help the district move toward our goal of providing Free Breakfast.
Free Breakfast will also require all families to have a positive account balance. We appreciate the effort made by most to move in this direction, but we still have a few families left that are keeping Free Breakfast from being a reality. We are currently setting up payment plans to help as much as possible. Please make arrangements to have all negative balances brought up to current.
